• Fill Out Form: Fill out the form on the following page and submit it along with a digital copy of your receipt of purchase of
eligible product from either (1) a 3M Authorized Distributor or (2) a distributor who purchased that product through a 3M
Authorized Wholesaler.
• Redeem: Confirm your purchase and receive your free product by sending the email with attachment(s) to n95redemption@mmm.com.
• Limit: 5 free cases of 8511 and/or 5 free fit test kits per customer. For example, if 15 cases of 8511 N95 80/ea are purchased, 5 cases
are eligible to redeem. If 30 cases are purchased, redemption max remains 5 cases.
